Please complete the online Work Order Request Form below and fill in, at minimum, all of the required fields. UTORid login credentials are required to access the Work Order Request form, which is mobile and computer friendly.
Regular Maintenance Requests may be submitted by Staff, Faculty and Students, and do not require authorization. Staff, Faculty and Students are highly encouraged to submit Work Order Requests for any building deficiencies noticed while on campus, regardless if they directly work in the area.
Move Requests and Facility Upgrades are billable to the requesting party, and will require authorization by the responsible Supervisor/Manager of the area. These requests will need to be submitted by a Supervisor/Manager, and must include FIS Billing Information (CFC, CC/IO, Fund).
