Letter Award Nomination Form

Before completing the nomination form below, please review the nomination package found here.
Please note: All nominations must be submitted through this form by Friday, March 2nd at 11:59 p.m. Nomination packages will NOT be accepted via email or in hard copy, EXCEPT:

  • Reference letters, which must be emailed in separately from this nomination form (see the “Section 4: References” below for more information);
  • Nominees that require alternative methods for submitting their nominations can arrange to do so by emailing leadership@utsc.utoronto.ca with their concerns. Please ensure if you need alternative means to submit your nomination package that you reach out before Friday, March 2nd 2018 at 5 p.m.

Section 4: References

Each nominee must have two (2) reference letters submitted on their behalf by a U of T community member (student, faculty, or staff). More information regarding references can be found here.

References must be submitted via email to leadership@utsc.utoronto.ca directly by the referee (NOT by the nominee or a third party). References are due by Friday, March 2nd 2018 at 11:59 p.m.
