Anyone with a suspected concussion should seek medical services. As a U of T Scarborough student, you can visit the Health & Wellness Centre, at SL270, to be assessed by a doctor. You can also visit your family doctor, local walk-in clinics, urgent care centres or Emergency Department at the nearest hospital.

Forms you need to bring with you:

  • Verification Of Illness or Injury Form – To be completed by your doctor if your concussion related symptoms will impact you for a short period of time (less than a week) and/or you require support for a petition, missed test/exam etc.
  • AccessAbility Services Medical Certificate – To be completed by your doctor if your concussion related symptoms will impact you for an extended period of time (more than a week) and you require academics accommodations and supports from AccessAbility Services.

Please be advised that some doctors off-campus charge for forms. Based on your discussion with your doctor and the severity of your concussion, you can decide which form(s) you require to be completed.

If you witness a person sustain a head injury and they become unconscious, call an ambulance.
Do not move the person or remove any equipment, such as a helmet or seat belt, in case of a spine injury.
