If you think you’ve had a concussion, please seek medical services. As a U of T Scarborough student, you can visit the Health & Wellness Centre, at SL270, to be assessed by a doctor. You can also visit your family doctor, local walk-in clinics, urgent care centres, or Emergency Department at the nearest hospital.
Forms you need to bring with you:
- AccessAbility Services – “Certificate of Disability” – To be completed by your doctor if your concussion related symptoms will impact you for an extended period of time (more than a week) and you require academics accommodations and supports from AccessAbility Services.
Please be advised that some doctors off-campus charge for forms.
