Cancellation
If you need to cancel an appointment, be sure to follow the cancellation policy to avoid any fees – and to free up time for other students using the clinic.
- The fee for missing a medical or counselling appointment is $30.
- The fee for cancelling a psychiatrist appointment with less than 24 hours notice is $100 for an initial appointment and $50 for a follow up.
Request for Records
- Request for health records to be sent to a health care professional in the community, $25
- Personal request for health records, $25
All applicable forms must be completed and fees must be paid prior to the request being fulfilled. Further information may be required in some instances prior to request being fulfilled.
